Our Active Health Exercise Referral scheme is a great way for people with health conditions to increase physical activity levels and improve health. Whatever your age, there is strong evidence to suggest that a physically active lifestyle can help you leade a healthier and happier life.
The 12-week scheme works with local healthcare professionals to provide a service where inactive, local residents can be referred to a scheme that encourages and gives you the tools to make lasting health improvements.
The activities available include gym and swimming, with weekly drop-in sessions provided by a qualified Exercise Referral instructor. Regular physical activity can benefit your heart health, manage your weight, improve your muscle and joint functions, and positively affect your mind. Not only will it help you manage and control existing conditions, it can help prevent further health complications developing.
Who can benefit?
The scheme is available to anyone aged 16+ who suffers from any of the following conditions:
- Osteo / Rheumatoid Arthritis
- Joint Replacement
- Simple Mechanical Back Pain
- Mental Health disorders (Stress, Anxiety and Depression)
- Coronary Heart Disease/Angina Pectoris
- Diabetes Type I and II
- Neurological Disorders
Discounted rates are offered to clients on the programme and further incentives are given to encourage them to continue exercising after the 12 week scheme.
How the Exercise Referral Scheme operates
To get started you will need to speak to your healthcare professional, such as your GP, who can refer you by issuing you with an Exercise Referral form. The referral form gives details of any medical conditions which will need consideration when the Instructor is creating your exercise programme. Once you have received your Exercise Referral form please contact your local club to arrange an Initial Assessment.
Your first visit to the club will be in the form of an Initial Assessment. This gives you the opportunity to meet your Exercise Referral Instructor and become familiar with the facilities. You will need to complete additional starter forms to enrol onto our scheme, as well as a review of your medical needs and preferences. In addition to this several physiological measurements will be taken (these are repeated after 6 and 12 weeks to assess your improvement). A follow-up appointment will then be arranged to go through your exercise programme.
At this appointment an exercise programme will be created for you by the Exercise Referral Instructor. The Instructor will take you through the different exercises, sets and reps ensuring that the technique is correct and you are confident completing the programme independently. This programme will last for 6-weeks at which point another appointment will be made to assess your progress and update your programme for the remaining 6 weeks. Further support is available through our weekly drop-in sessions. The sessions are led by our Exercise Referral instructor and provide an opportunity to receive guidance with your exercise programme, ask any questions or try out some new exercises.
